COMMUNICATION POLICY:
Sensitive information will only be communicated face to face by a medical practitioner or other appropriate health professional, unless there are exceptional circumstances.
Each Doctor will determine whether a patient requires a face to face consultation or if it is appropriate to communicate by electronic means. The practice’s policy on patient electronic communication and the cost is to be made available to patients. Electronic communication processes comply.
Our patients are given the option of being contacted by electronic means such as SMS. All new and existing patients in the practice are able to ask for a copy of our electronic communication policy and are asked to provide signed consent to agree or disagree to be communicated with in this manner.
The new patient consent form states that the practice may use this mode of communication: to send reminders for a scheduled appointment.
APPOINTMENTS:
You can see the doctor by appointment Monday to Friday. You can book online or over the phone. We will do our best to accommodate for the day and time that best suits you. Our consultation time is usually 10 minutes, please let the reception know if you require a long consultation. If you are unable to attend your appointment, please let us know at least one day in advance.
FEES & BILLING:
H&M Medical Centre is a bulk billing practice. You will need to have your medicare healthcare card for your convenience. Repeat scripts can be done without an appointment. Patients must be seen regularly, and if not seen in the last 6 months, an appointment will need to be made.
MEDICAL CERTIFICATES:
Medical certificates will only be provided by the doctor if a patient is seen and assessed, and only for the condition listed on the certificate. We do not back-date medical certificates.
PRESCRIPTIONS & REFERRALS:
Prescriptions are only written with consultation to determine the appropriate medications and dosages at the time. You will need to book for an appointment to see the doctor for specialist referrals. Referrals to specialists cannot be provided without prior consultation.
PATIENT RIGHTS & RESPONSIBILITIES:
At our practice, we do our best to cater to all our patients needs and ensure the best outcomes. Patients should notify the doctor if they decide to refuse a specific advice or procedure.
If you have a feedback and comment you want us to know about. Please feel free to speak with us or alternatively complete a questionnaire form which is located on the front counter and place in our suggestion box.
TEST RESULTS:
If you require a diagnostic test, it is recommended that you book a follow up appointment prior to leaving the centre. This way your doctor can explain and discuss the results with you as well as review any care plans. If you need to come in urgently after a test result, we will contact you directly.
EMAIL POLICY:
H&M Medical Centre does not communicate with Patients about their medical conditions through email. Unless Patients request results or medical certificate to be emailed to them.
